INDEMNITY FORM

I understand and certify that my participation in the internship is completely voluntary.

I acknowledge that the above company, Taylor’s University could not guarantee that the participants, equipment or activities would be free of hazard, accidents and / or injuries.

I voluntarily assume and accept the risk of taking part in such an event/internship and I, together with my heirs, executors, parents, guardian and administrators, hereby release the above mentioned company, Taylor’s University, its directors, its officers, students, staff and other representative, from any duty or care towards me, in connection with internship or in any related activities irrespective of whether such claim or claims arise through negligence of any person, or from any of the risks, dangers or hazards inherent in this event/internship, or any loss of, or damage to, any property from any cause whatsoever and I further indemnify and hold harmless associated persons against any claims howsoever the same may arise.

*Wherever mentioned Taylor’s University refers to Taylor’s University Sdn. Bhd.
